What is complex care?

All care is, to some extent, complex because each person is different, and each day is different we are bound to encounter complexities. So when people talk about and procure complex care what do they mean?

Complex care refers to a comprehensive and coordinated approach to healthcare that is designed for individuals with multiple and often chronic medical conditions. It involves the management and coordination of care across various healthcare providers and settings to address the complex needs of these individuals.

Complex care typically involves a team-based approach, with healthcare professionals from different disciplines working together to provide integrated care. This team may include primary care physicians, specialists, nurses, social workers, pharmacists, and other healthcare providers.

The aim of complex care is to improve health outcomes and quality of life for individuals with complex medical conditions by addressing their unique needs and challenges. This may include managing multiple medications, coordinating appointments and referrals, providing education and support for self-management, and addressing social determinants of health, such as housing, transportation, and access to social services.

Complex care often focuses on proactive and preventive measures to avoid hospitalizations and emergency room visits, as individuals with complex conditions may be at higher risk for these events. It also emphasises patient-centred care, taking into account the individual’s goals, preferences, and values when making healthcare decisions.

Overall, complex care is a patient-centred, multidisciplinary approach that aims to improve the overall well-being and health outcomes of individuals with complex medical needs by providing comprehensive, coordinated, and personalized care.
